Easy-to-Use Sponsorship Form Template
It works as a sponsorship agreement form, an event sponsorship form, or a sponsor commitment form for conferences, festivals, sports leagues, charity runs, and community events. The form has three short pages.
On the first page, the sponsor gives their company name, a contact person, a phone number, an email address, and a business address, and uploads the company logo you’ll use in promotion.
The second page opens with your event details and sponsorship packages in an editable text block. Add the event name, date, venue, expected attendance, and what each package includes.
The sponsor then chooses a package, from Platinum to Bronze, or a custom package. They tick how they’re contributing (cash, in-kind goods or services, or media and promotion). If they tick in-kind, a box opens for them to list what they’re providing.
There’s an optional box for recognition requests, like how the company name should appear on signs or which booth location they’d prefer.
The sponsor also picks how they’d like to pay, by invoice, bank transfer, or card payment link, and whether to pay in full or in two installments. The last page shows your sponsorship terms, including payment timing, logo use, and cancellations. The sponsor ticks a box to agree, signs, and adds the date.
