Do you want residents to pay their estate dues and levies online, without your treasurer chasing payments door to door? This template lets homeowners pay for one month or several at once, add any levies they owe, and pay through Paystack.

Why Use This Template

This template fits homeowners associations, residents’ associations, and gated estates that collect monthly dues and one-off levies for security, maintenance, and shared projects. It also helps residents who’ve fallen behind, since they can clear several months of arrears in one payment. Every payment comes with the house number and months covered, so your treasurer can update the dues register without asking for proof of payment.

What’s in the Homeowners Association Dues Form

  • House No. (Single line text) – Ties every payment to a property, which is how most estates keep their dues register.
  • Member type (Dropdown) – Separates ordinary members, executives, and life members, so the right dues apply to each.
  • Monthly dues (Single Item) – Comes with a quantity picker, so residents can pay for one month, several months ahead, or any arrears they owe.
  • Months covered (Single line text) – Lets residents say which months they’re paying for, so your treasurer knows exactly where to record it.
  • Levies (Checkbox Items) – Covers one-off charges like the Development Levy, Security Levy, Special Project Levy, and Welfare Contribution, each with its own amount.
  • Note to the treasurer (Paragraph text) – Gives residents space to explain a partial payment, a query about their balance, or a change of occupant.
  • Total (Total) – Adds up dues and levies so residents can check the amount before checkout opens.
  • Name (Name) – Lets you personalize follow-up without requiring a full account or login.
  • Email (for your receipt) (Email) – Paystack needs it to process the payment, and it’s where the resident’s receipt goes.
  • Phone (Phone) – Gives your treasurer a quick way to reach residents about arrears or upcoming levies.

How to Create a Homeowners Association Dues Form in WordPress

The easiest way to create an HOA dues form is with WPForms. Select the Paystack Homeowners Association Dues Form Template from the template library. Then use the drag-and-drop builder to add, remove, or relabel any field, including the member types and levies.

Next, connect your Paystack account with the Paystack addon and turn on payments in the builder’s Payments tab. When a resident submits the form, Paystack’s secure checkout opens so they can pay by card, bank transfer, USSD, or mobile money, depending on your country. WPForms doesn’t see or store card details, and they’re not saved anywhere in your WordPress database.

Finally, set up your Notifications and Confirmation message. Then embed the form on your estate’s website or share the link in your residents’ WhatsApp group.

Customizing Your Homeowners Association Dues Form

  • Match the member types to your estate. Many estates use Landlord and Tenant instead, since the two often pay different dues.
  • Rename the levies to what your estate actually collects, such as Waste Disposal, Power / Diesel, or Road Repair.
  • Set your currency before you set prices. Switch it in WPForms › Settings › Payments, then update the dues and levy amounts.
  • Offer auto-pay for residents who’d rather not remember every month. Recurring payments work with debit and credit cards.

Receiving HOA Dues Notifications

Once your form is live, you’ll want to know as soon as dues come in. In WPForms, you can email your treasurer about every payment and show residents a confirmation message with what they paid. Residents get a receipt to keep as proof of payment, and your treasurer gets the house number, months covered, and levies in the same email, ready to update the register.
