Do you want your members to give their tithes and offerings online, without chasing bank transfer screenshots or counting cash after service? This template lets them give to several funds in one go and pay through Paystack by card, bank transfer, USSD, or mobile money.
Why Use This Template
This template fits churches and ministries of every size, from a single parish collecting weekly offerings to a larger ministry raising money for a building project. It also fits members who travel, work weekends, or live abroad and still want to give on time. Each giving type has its own amount, so your finance team can see exactly what came in for tithes, offerings, and special projects without sorting through notes.
What’s in the Church Tithes & Offerings Form
- What are you giving today? (Checkbox) – Lets givers pick one or more giving types: Tithe, Offering, Thanksgiving, First Fruit, Seed / Pledge, Building Fund, or Other.
- Amount fields (Single Item, one per giving type) – Each one only appears when its giving type is ticked. Members can pay a tithe and an offering together, and you still see each amount separately.
- In honour of / occasion (Single line text) – Captures a thanksgiving occasion or the person someone is giving for, so your team can acknowledge it.
- Prayer request or note (Paragraph text) – Gives members a private way to share a prayer need. Only your pastoral team sees it.
- Total (Total) – Adds up every amount so givers can check the figure before they pay.
- Name (Name) – Lets you personalize follow-up without requiring a full account or login.
- Email (for your receipt) (Email) – Paystack needs it to process the payment, and it’s where the giver’s receipt goes.
- Phone (WhatsApp) (Phone) – Gives you a number to reach givers on WhatsApp, where many members prefer to hear from their church.
- Send me church updates on WhatsApp (Checkbox) – Builds a list of members who have agreed to receive announcements.

How to Create a Church Tithes & Offerings Form in WordPress
The easiest way to create a church giving form is with WPForms. Select the Paystack Church Tithes & Offerings Form Template from the template library. Then use the drag-and-drop builder to add, remove, or relabel any field, including the giving types.
Next, connect your Paystack account with the Paystack addon and turn on payments in the builder’s Payments tab. When a giver clicks Give Now, Paystack’s secure checkout opens so they can pay by card, bank transfer, USSD, or mobile money, depending on your country. WPForms doesn’t see or store card details, and they’re not saved anywhere in your WordPress database.
Once that’s done, set up your Notifications and Confirmation message. Then embed the form on your giving page, or share the link in your church WhatsApp groups.
Customizing Your Church Tithes & Offerings Form
- Rename the giving types to match how your church talks about giving. Some churches treat tithes and offerings as one fund, and others add Welfare, Missions, or Harvest.
- Add a Branch or Parish dropdown if you have more than one location. Then use conditional notifications to send each gift to the right treasurer.
- Offer monthly giving for members who’d like to give automatically. Recurring payments work with debit and credit cards.
- Keep the list to the funds members actually give to. Seven options is plenty, and a longer list means more scrolling on a phone.
Receiving Giving Notifications
Once your form is live, you’ll want to know when gifts come in. In WPForms, you can set up email notifications for your finance team and a confirmation message for givers. Every member gets a thank-you and a receipt as soon as they give, and your team can see what came in for each fund right away.
