  • Likert Scale for Likelihood: Another table asking how likely they are to buy again, recommend your product, and recommend your company. Same five-point scale.
  • Open Comments: If a customer gives you a low star rating, the form will use smart conditional logic to show a text field where they can explain what went wrong.

Customizing the Form Template

The great thing about using a survey form to ask people hard-hitting questions is you never have to guess at how people feel. Trust us, they’ll tell you one way or another whether you’re giving them what they need or expect.

  • Keep it short: People abandon long surveys. Aim for 5-10 questions maximum unless you’re offering an incentive for completion.
  • Mix question types: Structured questions (ratings, Likert scales) are easier to analyze. Open-ended questions provide context. Use both.
  • Ask one thing per question: “How satisfied are you with our product quality and customer service?” is actually two questions. Split them.
  • Use consistent scales: If one question uses 1-5 stars and another uses 1-10, comparing results gets confusing. Stick with one scale type.
  • Add logic for better data: Show follow-up questions based on ratings. If someone rates you 5 stars, ask what you should keep doing. If they rate you 2 stars, ask what needs to change.
