Customizing the Form Template

- Keep it short: People abandon long surveys. Aim for 5-10 questions maximum unless you’re offering an incentive for completion.
- Mix question types: Structured questions (ratings, Likert scales) are easier to analyze. Open-ended questions provide context. Use both.
- Ask one thing per question: “How satisfied are you with our product quality and customer service?” is actually two questions. Split them.
- Use consistent scales: If one question uses 1-5 stars and another uses 1-10, comparing results gets confusing. Stick with one scale type.
- Add logic for better data: Show follow-up questions based on ratings. If someone rates you 5 stars, ask what you should keep doing. If they rate you 2 stars, ask what needs to change.
