How the Order Form Works

Product-based businesses, artisan makers, small retailers, wholesale suppliers, and anyone selling physical goods without a full online store will find this template really useful:

Product Selection:

  • Visual Product Displays: Three product boxes with images (shown as package icons in the template) displaying “Designer – $0.00” as placeholders.
  • Product Selection Fields: Three separate dropdown fields for “First Item – Product Name”, “Second Item – Product Name”, and “Third Item – Product Name” where customers choose what they’re ordering.
  • Quantity and Color Fields: For each product slot, customers can specify quantity and color preferences.

Total Calculation:

  • Total Amount Display: Shows “$0.00” initially, but automatically calculates the order total based on products selected and quantities chosen.

Customer Information:

  • Name: Captures first and last name for order processing and shipping labels.
  • Email: Creates your communication line for order confirmations and shipping notifications.
  • Phone Number: Provides a contact method if delivery issues arise.
  • Shipping Address: Collects complete delivery details including street address, city, state/province, country, and postal code.
  • Delivery Instructions (Paragraph Text): Lets customers add notes about gate codes, delivery preferences, or special handling.

Payment Processing:

  • PayPal Commerce Field: Handles the transaction for the complete order total.

Payment Options Available

Customers can pay using PayPal, credit or debit cards entered directly into the form, or Apple Pay, Google Pay, and Venmo depending on what’s available in their region. The full order amount processes in a single transaction.

For Basic/Plus users, a 3% platform fee applies to one-time payments. Upgrade to WPForms Pro or higher to remove this fee and unlock additional features with the PayPal Commerce Addon.

Making This Form Match Your Products

  • Product images and pricing: Replace the placeholder box images with actual product photos. Update the pricing shown in each product display to match your real prices.
  • Number of items: The template shows three product slots. If you only sell two products, delete the third section. If you need more, duplicate a product section and adjust the field names accordingly.
  • Product variants: The template includes “Product Color” dropdowns. Change these to whatever variants you offer. Maybe “Size”, “Flavor”, “Material”, or “Edition”.
  • Quantity limits: If certain products have maximum order quantities, add descriptions or set field validation to prevent over-ordering limited items.
  • Pricing structure: If you offer volume discounts or wholesale pricing tiers, add conditional logic (available in Pro plans) to adjust the total based on quantity ranges.
  • Shipping options: Consider adding a dropdown where customers select shipping speed (Standard, Express, Overnight) with different costs associated. Connect this to your total calculation.
  • Order date or delivery date: Some businesses need to schedule deliveries or production. Add a date picker field where customers specify when they need the order.
  • Custom requests: If you accept personalization or custom specifications, expand the delivery instructions field or add dedicated fields for engraving text, custom colors, or special requests.

Connect your PayPal Business account via WPForms » Settings » Payments, import this template, then embed it on your products page or dedicated order page.
