What’s in the Emergency Contact Form Template
- Member Details: Name, date of birth, email, phone, home address, and an optional box for medical conditions, allergies, or medications.
- Primary Emergency Contact: Name, relationship, mobile phone, and an alternative phone.
- Second Emergency Contact: An optional second contact that only appears when the member wants to add one.
- Contact Consent: A checkbox confirming the contacts know they’re listed.
You’ll get an email with each new entry, and the member gets a copy of what they sent. Ask members to fill it in again each season or term so your records stay current. Medical details are sensitive, so check the privacy rules for your area and only ask for what you need.
