Contact forms are the usual way to reach a business, but they rarely show where that business is. People who want to visit, drop something off, or check you’re nearby have to leave the page and look you up.
The Contact Form with Map Template puts an interactive map of your business at the top of a short contact form. With one location, it’s a find us map next to your contact details. With several, visitors see every branch, office, or store on one map and pick the one they’re writing to, so you know which location each message is about.
It suits local shops, clinics, salons, law and accounting offices, franchises, and any business with more than one site. You may also know it as a contact form with Google Maps, a find us form, or a multi-location contact form.
Exploring the Contact Form with Map Template
The map comes from the Map field in the Geolocation addon. You add your locations in the builder, and visitors see them with your own markers. Here’s what’s in the form:
- Which location are you contacting?: An interactive map of your locations with their names and addresses listed below it. Visitors pick one, and their choice is saved with the entry.
- Name and Email: Who’s writing and where to send your reply.
- Subject: A short summary of their question.
- Message: The details of their question or request.
To set up the map, add each location’s name and address, then choose a marker icon or upload your own image, with a color and size. With a single location, set the zoom level and turn off location selection. With two or more, the map zooms to fit them all. You can hide controls you don’t need, like street view or full screen, for a cleaner look. The map needs the Geolocation addon connected with a valid API key, or it won’t load.
Each message arrives by email, and hitting Reply goes to the visitor’s address. You can rename the fields, add a phone number, or edit the confirmation to include your opening hours.
