Once your audience grows, your inbox fills with brand pitches, collaboration ideas, podcast invites, and reader questions, all mixed together. This Kit Signup Contact Form Template sorts them before they reach you and lets readers join your newsletter while they’re there.
The form asks for a name and email, then what the message is about: a sponsorship or brand deal, a collaboration, a podcast or speaking invite, or a question. Add Other Choice covers anything else. Company or brand and a website or social profile are optional, so readers can skip them and brands can fill them in. An unticked newsletter box comes last.
How the Kit Signup Contact Form Template Works
When someone ticks the newsletter box, the WPForms Kit addon (Kit was formerly ConvertKit) subscribes them to your Kit account. You can tag them, for example “contact-form”, and save their inquiry type to a Kit custom field, so a reader who asked a question and a brand that pitched a sponsorship don’t get the same emails. You can also link the connection to one of your Kit forms. Conditional logic on the connection means only people who tick the box are subscribed.
It works as a creator contact form, a sponsorship inquiry form, a brand collaboration form, or a media kit contact page. It suits bloggers, YouTubers, podcasters, newsletter writers, and course creators.
You’ll get an email for each message, set up so you can reply straight to the sender, and they’ll get a copy of what they sent. Edit the inquiry types to match the work you take on.
The Kit addon comes with WPForms Plus and higher.
