Agreement
Clear Signature

The Vendor Agreement Form Template gives your business one place to collect a supplier’s details and their signature on your standard vendor terms before you place the first order.

How Does the Vendor Agreement Form Template Work?

You add your vendor terms to the text block once, covering pricing, delivery, invoicing, payment timing, quality, and insurance. Each new vendor, supplier, or distributor then fills in what they’ll supply and their business details, agrees to your terms, and signs online.

This template needs WPForms Pro because it uses the Signatures addon. You might also know it as a supplier agreement, a vendor contract, a supply agreement, vendor terms and conditions, or a vendor onboarding agreement.

Fields in the Vendor Agreement Form Template

  • What You’ll Supply
    • Goods or Services Supplied: What the vendor will provide, in their own words.
    • Quote or Price List Reference: The quote, catalog, or price list behind their pricing, so you can check invoices against it.
    • Earliest Delivery or Start Date: When the vendor can first deliver or start work.
  • Vendor Information
    • Business Name: The vendor’s registered or trading name.
    • Contact Name: The person you’ll deal with day to day.
    • Job Title: The signer’s role, since they’re signing for the business.
    • Email and Phone: Where to send orders and questions.
    • Business Address: The vendor’s main location, with fields that work for any country.
    • Insurance Certificate: An optional upload for when your terms ask for proof of insurance.
  • Vendor Terms
    • Terms: Your vendor terms, with bracketed placeholders for payment timing, delivery, quality, insurance, and how either side can end the agreement.
    • Agreement: A required checkbox confirming the signer can act for the business and agrees to your terms.
    • Vendor Signature and Date Signed: The vendor’s e-signature and the date.

Each signed agreement sends you an email notification, and the vendor can get a copy too. Add the PDF addon to keep every agreement on file as a PDF you can countersign. Supply terms are treated differently from place to place, so have yours reviewed for your area.
