What Goes on a Staff Appraisal Form Template?
Think of staff appraisals like check-ins. You want an idea of how things are going at certain agreed-upon intervals. During these meetings, the supervisor asks the employee how things are going, discusses their workload, and gets a general sense of how the employee is feeling.
Between this meeting and regularly monitoring the employee’s work, the supervisor should be able to accurately fill out the form.
An appraisal form must collect the employee’s name and email, as well as the supervisor’s name and email. From there, the supervisor will have to rate the employee on key skills.
