If the participant is under 18, a parent or guardian signs here.

Before someone joins a climb, a paddle, a ride, or a class you run, you want them to understand the risks and sign a release. This Release of Liability Form Template puts your waiver online, so participants can sign before they arrive and parents can sign for their kids.

It works for adventure sports, tours, camps, sports clinics, and one-off events. You might call it a liability waiver, an activity waiver, a participant waiver, an assumption of risk form, or an accident waiver.

Using the Release of Liability Form Template

Participant details come first: name, date of birth, phone, email, and address.

A yes or no question then asks whether the participant is under 18. The answer decides who signs.

Participants choose the activity they’re signing for from your list and add the date they’re taking part.

An optional health section asks about conditions, allergies, and medications, so your staff know before the day. People only fill it in when something applies.

The emergency contact’s name and phone number are required, so you always know who to call.

Your waiver terms appear in an editable text block, with bracketed spots for your business name, location, and the risks of your activity. Three separate checkboxes then record the participant’s assumption of risk, the release, and indemnification.

Adults sign for themselves. For a minor, the form asks for a parent or guardian’s name, and they sign instead. The date signed comes last.

You’ll get an email for every waiver, and the signer gets a copy. Add the PDF addon if you’d like a PDF of each signed waiver. Waiver rules, especially for minors, vary by location, so have your terms reviewed for your area.
