Do you want to record the ongoing attendance for a class, group meeting, or workshop with one simple form? Then you need the Recurring Attendance Form Template from WPForms.
How Does the Recurring Attendance Form Template Work?
First, your form user provides a bit of contact information–their name and email address. This user might be an instructor, manager, or other person taking attendance, so you may want to edit your attendance form to reflect their role.
Then, the Recurring Attendance Form Template harnesses the power of the Likert Scale field to list attendee names and dates in rows and columns.
On the left side of the scale, we’ve included 10 rows for you to customize with the names or labels of each attendee, which could be students, employees, or event participants.
Then, we’ve included columns for five days of the week to get you started. You might customize these labels running across the top with days, weeks, or specific dates and event names. We suggest using 8 columns or less here to keep the formatting intact.
Next, this scale has been duplicated to cover 3 weeks’ worth of daily attendance. You can continue duplicating the scales and editing the columns for additional attendance, or delete the scales you don’t need. It’s easier than ever to customize this form how you see fit!
As for functionality, your users can take attendance as many times as they need before submitting the form. With the Save and Resume Later option at the bottom, your form user simply saves the form after taking attendance until they’re set to send the full report. They can include any additional notes to submit with their attendance, too.
