What Should a Photo Consent Form Include?

On our template, we’ve included the following:

    • Conditions of the consent
    • Individual’s information
    • Description of the image(s)
    • Revocation (terms of cancelation of the consent)
    • Signature and date field

A revocation clause is vital to include in most consent forms. The revocation on this photo release form lets the releasee choose between never revoking the consent or withdrawing consent in writing.

We’ve also added a signature field (available to Pro and Elite WPForms subscribers) to have the releasee sign the document. Add a second signature field to the photo consent form and ask the releaser to sign.
