When a filmmaker or brand wants one of your tracks, you need to know the project, where it will be shown, and for how long, then get paid before you send the files. This template handles the request, the signature, and the fee in one form.

How Does the Music Licensing Agreement Form Template Work?

It’s built for independent musicians, composers, and production music libraries. Use it as a music license agreement, a sync license, a master use license, or a simple form for licensing a song for video, ads, podcasts, and games.

First, the licensee gives their name, company or production, email, phone number, and mailing address, so you know who’s licensing the track and where to send the paperwork.

In the Track and License section, they name the track and pick the license types they need: synchronization, master use, mechanical, or performance. Then they give the project title, choose the project type, and describe how the music will be used.

They also pick the territory and the license term. Your license terms appear in an editable text block that covers what’s included and what isn’t, the fee, credit, and ownership. The licensee ticks a box to agree, signs, and adds the date.

Last, they enter the fee from your quote and pay by card through Stripe. You get a notification, and the licensee gets a copy for their records. Add the PDF addon if you’d like to send each license as a signed PDF.
