Types of Customer Acknowledgement Forms

There are various uses for customer acknowledgement forms.

First, client permission forms. These forms are generally used to cover medical issues. Some airlines ask you to submit one due to the risk of contracting diseases onboard the flight. Spa and wellness centers often request they’re filled out beforehand to release them of any liability before a customer has their treatment.

Another type of customer acknowledgement form is a client receipt form. This form is a legal acknowledgement that the file, service, funds, or property was received.

The other popular form type is the client acknowledegment of rights. This form is usually written by an attorney, and the client will be informed what their legal rights are. There will be a box to agree to the terms and a section to sign the form like the others above.
