This Address Autocomplete Contact Form Template suggests real addresses as visitors type and fills in the street, city, region, postal code, and country for them. A small map under the field shows the pin, so they can check it before they send. It’s built for home services, cleaners, landscapers, movers, installers, mobile groomers, local delivery, and anyone who gives on-site quotes. You may also know it as an address autofill form, a service request form, or a home visit request form.
Using the Address Autocomplete Contact Form Template
Visitors enter their name, email address, and phone number, then start typing their service address and pick it from the suggestions. The last field asks what they need, so you know the job before you call back. Every field is required, because a visit needs an address and a way to reach someone.
Autocomplete comes from the Geolocation addon, using Google Places or Mapbox. You’ll add an API key from one of them in the addon’s settings, and Google needs your site to run on HTTPS. The address field uses the international format, so it works outside the US too.
Your notification includes the full address, and hitting Reply goes to the visitor’s email. The visitor gets a copy of their request too. You can rename the fields, add a preferred date, or turn off the map if you don’t need it.
