Some messages shouldn’t be sent until the sender has seen your terms. A law firm wants people to know that a contact form doesn’t make them a client. An agency that takes pitches wants its submission policy accepted first, and a freelancer may want quote requests to follow their terms of service.
This Terms of Service Contact Form Template adds a required checkbox to a simple contact form. Visitors read a short summary of your terms or disclaimer, follow a link to the full version, and tick the box before they can send.
Who Needs a Terms of Service Contact Form?
It works for solicitors and attorneys, financial advisers, consultants, agencies, publishers, and online services that want acceptance on record before a conversation starts. You might also call it a terms and conditions contact form, a contact form with a T&C checkbox, or a disclaimer contact form.
The form asks for the visitor’s name, email address, and message, then shows the terms checkbox last. The checkbox text includes the date your terms were last updated, so each entry shows which version the person agreed to. Replace the bracketed text with your own summary and a link to your terms page.
You’ll get an email with each message, with the reply-to set to the visitor’s address.
A checkbox records that someone agreed, but it doesn’t decide whether your terms hold up. Have your terms and disclaimer reviewed for your area.
