What Goes on a Teacher Evaluation Form Template?

Administrators typically evaluate teachers a few times a year using some type of standardized form. The form includes questions or fields about a teacher’s planning, content area knowledge, classroom environment, instructional presentation, and professionalism.

The details and questions might differ from school to school, but the general topics remain the same.

Our teacher evaluation form template is split into different sections. Each one asks the observer to evaluate the teacher based on the criteria listed. Likert Scales are really useful for evaluation forms like this. They allow the evaluator to answer based on the degree to which they agree with a statement.

With our template, they evaluate the teacher based on whether they’ve observed a teaching method, trait, or quality as being evident, somewhat evident, or not evident. There’s also an option for “N/A” if the criteria don’t apply.
