What Goes on a Teacher Evaluation Form Template?

Administrators typically evaluate teachers a few times a year using some type of standardized form. The form includes questions or fields about a teacher’s planning, content area knowledge, classroom environment, instructional presentation, and professionalism.

The details and questions might differ from school to school, but the general topics remain the same.
