When one inbox gets every message, sales leads wait behind support tickets and billing questions sit with the wrong person. Someone has to read each one and forward it, and replies slow down.

This Select a Recipient Contact Form Template lets visitors choose the team they want to reach and sends their message straight to that team’s email address. Use it for a business with separate sales, billing, and support inboxes, a school or charity with several offices, or an agency where each department answers its own questions. You may know it as a department contact form, a multiple recipient contact form, or a contact form that sends to different emails.

How Does the Select a Recipient Contact Form Template Work?

Visitors enter their name and email address, choose Sales, Billing, or Support from a short list, and write their message. All four fields are required, so every message arrives with a team and a way to reply.

Behind the form, each team has its own notification with conditional logic. When someone picks Billing, only the Billing notification goes out, and the same goes for the other teams. Each notification sets the reply-to address to the visitor’s email, so the team can answer with one click. The visitor gets a copy of their message too, so they know it arrived.

To finish setup, add each team’s email address to its notification. You can rename the teams or add more, such as Press or Careers, and give each new one a notification of its own. Send a test message for each team before you publish to check it lands in the right inbox.
