What Is a Reading Program Book Log Template?
Libraries, schools, and even bookstores often hold reading programs throughout the year. The programs build literacy and encourage people to read on a regular basis. To track the books participants have read, a book log is often used.
Depending on the program, reading program book log templates can be simple or more complex. A simple log might just ask for the reader’s name along with the book’s title and author. A more complex log might ask for that same information, along with more thoughts from the reader.
Our reading program book log template uses conditional logic to dynamically serve the correct audience.
For example, if a parent or guardian is filling out the form for a child in the kids program, the form will only ask what the book is about and if the reader has a rating out of 5 stars.
However, if tweens, teens, and adults fill out the form, they’ll have additional questions about whether they’d recommend the book, what kind of reader would enjoy it, and if there are any content warnings they’d list.
Although not required, you could connect this form to a Google Sheet with the Google Sheets addon and keep track of the submissions that way.
