Some visitors put off a long contact form, and others hit send before they’ve checked what they wrote. This Multi-Page Contact Form Template splits a contact form into three short, named pages and ends with a review step, so people can check their message before it reaches you.

It works for service businesses, agencies, schools, and anyone who wants a few more details than a basic contact form without showing every field at once. Each page stays short on a phone screen, too. You might also call it a contact form with page breaks, a paged enquiry form, or a contact form with a progress bar.

How Does the Multi-Page Contact Form Template Work?

A progress indicator at the top shows the name of each page, so visitors always know how much is left:

  • Your Details: Name, email, and an optional phone number.
  • Your Message: A subject line and the message itself.
  • Review and Send: A preview of everything they’ve entered. They can go back and fix anything before they press Send Message.

You’ll get an email for each submission with the visitor’s address as the reply-to, and they’ll get a copy of their message. In the drag-and-drop builder you can rename the pages, add fields to any of them, or change the progress indicator style. If you’d rather show one question at a time in a styled card, try the Multi-Step Contact Form Template, which uses the Lead Forms addon.
