Looking for a customizable quiz template that automatically grades responses and delivers instant results? The Graded Quiz Template from WPForms makes it possible to easily create knowledge assessments, tests, training evaluations, or educational quizzes, without starting from scratch.
Understanding the Graded Quiz Template
Graded quizzes evaluate responses as either correct or incorrect and calculate a final grade based on the percentage of correct answers.
Unlike personality quizzes (where there are no wrong answers) or weighted quizzes (where answers have different point values), graded quizzes have one objectively correct answer per question.
Automatic Scoring System
The template comes with automatic scoring already configured. Here’s how it works:
- Each question is set to “Include in Quiz Scoring” by default.
- You designate one answer choice per question as correct (marked with a checkmark icon).
- WPForms automatically counts correct vs. incorrect responses.
- The final grade is calculated as: (Correct Answers ÷ Total Questions) × 100.
- Participants see their percentage score and corresponding letter grade instantly.
Pre-Configured Grading Scale
The template uses the standard academic grading scale:
- Grade A (90-100%): Answered 9-10 questions correctly
- Grade B (80-89%): Answered 8 questions correctly
- Grade C (70-79%): Answered 7 questions correctly
- Grade D (60-69%): Answered 6 questions correctly
- Grade F (0-59%): Answered 5 or fewer questions correctly
10 Pre-Structured Questions
The quiz includes 10 placeholder questions (Question 1, Question 2, etc.) each with:
- Multiple choice field type (the standard for graded quizzes).
- Three answer options (Answer 1, Answer 2, Answer 3).
- First answer pre-marked as correct (easily changeable).
- “Include in Quiz Scoring” enabled automatically.
- Required field validation.
Smart Tags for Dynamic Content
The template includes five outcome messages. One for each letter grade. Each outcome uses WPForms’ smart tags that display dynamic quiz data:
- {quiz_grade}: Letter grade (A, B, C, D, or F)
- {quiz_percentage}: Percentage score (e.g., 87%)
- {quiz_correct_answers}: Number of correct responses
- {quiz_incorrect_answers}: Number of incorrect responses
- {quiz_total_questions}: Total questions in quiz
- {quiz_summary_link}: Link to detailed results breakdown
Optional Feedback Settings
In the Settings tab, you’ll find options to customize the learning experience:
Show Correct Answers
Choose when (or if) participants see which answers were correct:
- Do not show the correct answer (default): Maintains assessment integrity.
- Show the correct answer as soon as a choice is made: Provides immediate learning feedback.
Custom Feedback Messages
Personalize the messages that appear after each answer:
- Correct Answer Message: Default: “Correct! Great job.”
- Incorrect Answer Message: Default: “Incorrect. The right answer is {quiz_correct_answer}”
These messages support smart tags, allowing you to include the correct answer, explanations, or learning resources.
Optional Time Limit
Add urgency or simulate test conditions by enabling a time limit:
- Set minutes (1-60) and seconds (0-59)
- Choose what happens when time expires:
- Submit quiz automatically
- Lock remaining questions
- End quiz completely
- Customize the “time exceeded” message
