If people in the EU or UK can reach you through your website, you’ll usually need their clear consent before you store what they send you. A plain contact form doesn’t ask for it.

This GDPR Contact Form Template adds a GDPR Agreement checkbox to a simple contact form. Visitors tick it before they can send their message, and the wording tells them who’s keeping their details, why, and where to read your privacy policy. The box can’t be pre-ticked, and the agreement is saved with each entry.

It works for small businesses, freelancers, bloggers, and charities with European visitors. You might also know it as a GDPR consent form, a UK GDPR contact form, a privacy policy checkbox, or a data protection contact form. It only asks for what you need to reply.

Elements of the GDPR Contact Form Template

  • Name: First and last name, so you know who you’re replying to.
  • Email: Where your reply goes.
  • Message: Room for their question or request.
  • GDPR Agreement: A required consent checkbox with editable wording and a link to your privacy policy.

You’ll get an email with each new message, set up so you can reply straight to the sender. The confirmation tells them how to ask you to delete their details later. The GDPR Agreement field needs WPForms’ GDPR Enhancements setting, which is on by default in new installs and also lets you turn off user cookies and stored IP addresses.

A consent checkbox is one part of handling personal data, not all of it. Check what the GDPR and your local privacy laws ask of your site, keep your privacy policy up to date, and edit the consent wording to match how you really use the data.
