Serving a notice to a tenant is never fun, and it’s easy to miss a detail when you’re writing one from scratch. This Eviction Notice Form Template helps landlords and property managers put together a clear written notice, email the tenant a copy, and keep a dated record of every notice issued.
Use it as a notice to quit, a pay or quit notice for late rent, or a notice to vacate at the end of a tenancy.
Why Use an Eviction Notice Form Template?
The form starts with the tenant’s name, email, and the property address. Then you choose the reason for the notice, like nonpayment of rent, a lease violation, or the end of a tenancy. If the reason isn’t listed, select Other and type it in.
For unpaid rent, an Amount Owed section appears with the period owed, rent, late fees, and any partial payments. The total balance due is calculated for you, so the numbers on the notice add up.
You’ll also set the deadline for the tenant to pay, fix the issue, or move out, note how the notice is being delivered, and add your contact details. The notice is signed and dated at the end.
When you submit the form, you get a copy of the notice, and the tenant gets one too if you entered their email. Since this form is for your team, you can use the Form Locker addon to limit it to logged-in users.
Notice periods and requirements vary by state and country, so check the rules for your area or have a lawyer review your notice before you use it.
