Send Invoices to Someone Else?
For example, a PO number or the best time to call.

Once a new client says yes, you need the right details on file before the first invoice goes out. Pulling names, emails, and billing addresses out of old email threads is how invoices end up with the wrong person.

This Client Contact Details Form Template gives new clients one place to send their contact information, their billing address, and how they like to be contacted. If someone else pays the invoices, the client can add that billing contact too.

Using the Client Contact Details Form Template

It’s a good fit for freelancers, agencies, consultants, bookkeepers, photographers, and contractors who onboard clients online. You might know it as a client information form, a new client form, a customer information sheet, or a client details form.

Here’s what’s in the form:

  • Contact Details: The client’s name, company, job title, email, phone, and preferred contact method (call, text, or email).
  • Billing Details: A billing address with an international layout. There’s also an optional billing contact name and email, which appear only when invoices go to someone else.
  • Notes: A box for anything else, like a PO number or the best time to call.

Send the link with your welcome email or add the form to your onboarding page. You’ll get an email for each new client, and they’ll get a copy of their details. Need to know more about the project itself? Pair it with the Client Intake Form.
