People who contact you don’t all want the same emails. Some want your offers, some want your tips, and some only want event news. This Brevo Signup Contact Form Template lets them send a message, join your list if they’d like, and pick the topics they care about.
The form collects a name, email, and message, followed by an unticked email updates checkbox. Once someone ticks it, a short list of topics appears: news and updates, offers and discounts, tips and guides, and events.
How the Brevo Signup Contact Form Template Works
When the box is ticked, the WPForms Brevo addon adds the person to the Brevo list you choose. Their topic picks are saved to a Brevo multiple-choice attribute, so you can send each group only what they asked for. With Brevo’s double opt-in turned on, they get a confirmation email first and join the list only after they click it. Conditional logic on the connection keeps everyone else off the list.
Use it as a Brevo or Sendinblue contact form, a contact form with email preferences, or a newsletter signup with topic choices.
You’ll get an email for each message that you can reply to directly, and the sender gets a copy. Change the topics to match the emails you actually send.
