A contact form tells you someone wants to talk. It doesn’t tell you what about, and it doesn’t help you follow up with the people who’d like to hear from you again. This AWeber Signup Contact Form Template does both.
Visitors give their name and email, pick what their message is about, and write it. An unticked checkbox at the end lets them join your email list if they want to, and says they can unsubscribe at any time.
How the AWeber Signup Contact Form Template Works
The topic question gives every message a label you can act on: a general question, a quote request, a problem with an order, or a partnership pitch. Add Other Choice lets people describe anything else.
When someone ticks the email box, the WPForms AWeber addon adds them to the list you pick. You can apply a tag to everyone who signs up through this form, such as “website-contact”, and AWeber can start a campaign as soon as that tag is applied. Their topic can be saved to an AWeber custom field, too, so you can send quote requesters different emails from people who asked a general question. Conditional logic on the connection means nobody is added unless they tick the box.
It works as an AWeber contact form, a contact form with newsletter signup, or an AWeber opt-in checkbox for an existing contact page. It’s a good fit for service businesses, coaches, course creators, and online shops.
You’ll get an email for each message, set up so you can reply straight to the sender, and they’ll get a copy of what they sent. Edit the topics to match how your business works.
The AWeber addon comes with WPForms Plus and higher.
