Claiming a lower duty rate under the Australia–United States Free Trade Agreement (AUSFTA) means being able to explain why the goods qualify. There’s no official certificate of origin for AUSFTA, so importers and customs brokers usually ask the exporter or producer for a signed origin declaration and the shipping paperwork. This template collects both in one form.
It suits customs brokers, freight forwarders, and importers in Australia or the US who handle US–Australia trade, whether you call it an AUSFTA certificate of origin, an origin statement, or a supplier’s declaration for preferential tariff treatment.
The AUSFTA Form Template Explained
The form opens with the exporter’s details: name, phone, email, and address, plus the producer’s name if someone else made the goods.
Next come the importer’s name, phone, email, and address, so you can match the declaration to the right shipment and client.
The goods page asks for the product name, a full description, the HS code, the quantity, and the total invoice value.
On the origin page, the exporter picks the country of origin (Australia or the United States) and the origin criterion that applies: wholly obtained, made entirely from originating materials, or meeting the product-specific rule. There’s room for a reference number if the exporter has one, and a short note you can edit with your broker’s instructions.
The documents page takes the invoice number and the commercial invoice, with optional uploads for the bill of lading, packing list, and any origin statement or cost records.
The exporter finishes with a declaration checkbox, the signatory’s name and position, a signature, and the date. You get an email with every answer and the files, and the exporter gets a copy. Add the PDF addon if you want each declaration saved as a PDF.
Rules of origin can be detailed, so have your broker or a trade adviser check the criteria and document list before you rely on the form. The template is a starting point, not an official customs form.
