Think of them as the initial handshake in a business relationship. One that needs to be firm enough to establish credibility but welcoming enough that people want to continue the conversation.

In this post, I’m going beyond the usual templates and talking about some fresh, effective lead generation form examples that do more than just collect names and email addresses.

## What Is a Lead Generation Form?

A [lead capture form](https://wpforms.com/lead-capture-form/) (or lead gen form) is a web form built specifically to collect contact details and qualifying information from people who are interested enough in your offer to type something in.

![The Lead Forms addon for WPForms Pro](https://wpforms.com/wp-content/uploads/2023/03/Filling-out-Lead-Forms.gif)The goal is to turn a passive visitor into a known prospect you can follow up with, so they show up in different shapes depending on what you’re trying to capture and where the visitor is in their journey:

- **[Newsletter signup forms](https://wpforms.com/newsletter-signup-form/)**: Short forms (often just an email field) that subscribe visitors to recurring content. Lowest possible barrier to entry.
- **Demo request forms**: Longer forms used for higher-intent offers, usually 5 to 7 fields with at least one qualifying question.
- **Gated content forms**: Collect contact info in exchange for an ebook, report, template, or research.
- **Quiz and assessment forms**: Interactive multi-step forms that personalize a result and capture an email at the end.
- **Multi-step lead forms**: Long captures broken into smaller pages so they feel less overwhelming.
- **Sales inquiry forms**: Basic contact forms with a few qualification fields so the sales team can prioritize.

According to recent [lead generation statistics](https://wpforms.com/lead-generation-statistics-and-trends/), nearly half of marketers say web forms are their highest-converting lead capture tool, which makes the design choices below worth getting right.

You can recreate most lead forms using the [WPForms Lead Forms addon](https://wpforms.com/addons/lead-forms-addon/). It lets you create a form with a conversational layout that can be embedded anywhere on your WordPress site.

As soon as visitors land on the page, they’re guided by a simple question. “Where do you want to start?”

- Newfound presents clear, distinct choices like Selling a Home, Buying a Home, and Buying and Selling.
- The form uses a minimalist design with vibrant buttons that stand out against the background.

![pete and pedro lead generation](https://wpforms.com/wp-content/uploads/2019/08/pete-and-pedro-lead-generation-1024x927.png)Pete & Pedro’s lead capture form takes a fun, interactive approach by guiding users through a visual quiz to determine their ideal hair styling product. This method not only keeps visitors engaged but also helps them make confident purchasing decisions.

**Key Tactics:**

- Users can easily choose their preferred hair style, shine level, and hold strength, which leads to a personalized product recommendation.
- By breaking the form into segments, Pete & Pedro manages to reduce the feeling of overwhelming users with too many questions.

#### 28. Morning Brew

![morning brew newsletter form](https://wpforms.com/wp-content/uploads/2019/08/morning-brew-newsletter-form-e1778228709583-1024x469.png)Morning Brew’s newsletter signup is one of the cleanest lead capture forms on the internet. A single email field, a punchy CTA, and a piece of social proof above the form telling you how many millions of readers already get the newsletter every morning.

**Key Tactics:**

- Single field reduces friction to almost zero, which works because the offer (a free daily newsletter) doesn’t need much qualifying info upfront
- Social proof is front-loaded with a reader count, building credibility before the visitor even types
- The CTA copy uses voice and personality instead of a generic “Subscribe”, which fits the brand’s irreverent tone

#### 29. AG1

![AG1 lead generation quiz](https://wpforms.com/wp-content/uploads/2019/08/AG1-lead-generation-quiz-1024x577.png)AG1 (formerly Athletic Greens) uses a quiz-style multi-step form to capture leads and personalize the product experience at the same time.

By the time you finish answering questions about your goals and lifestyle, the form has both your email and enough context to send personalized follow-up.

**Key Tactics:**

- Quiz-style format makes the form feel like value (you’re getting a personalized recommendation), not a chore
- Each step asks one or two questions, so the form never feels long even though it captures more data than most
- The email capture sits at the END of the quiz, after the visitor has invested time, which raises completion rates

You can build something similar on your own site with a [lead generation quiz in WordPress](https://wpforms.com/how-to-create-a-lead-generation-quiz-in-wordpress/) using the WPForms Quiz addon.

#### 30. Figma

![figma contact us form](https://wpforms.com/wp-content/uploads/2019/08/figma-contact-us-form-1024x617.png)Figma’s demo request form is a model of B2B restraint. The fields are minimal (work email, company name, role, team size), the copy is direct, and there’s a clear “what happens next” expectation.

**Key Tactics:**

- Tells you exactly what happens after submit (“a member of our team will reach out within 24 hours”) to remove uncertainty
- Uses business-context fields (role, team size) to qualify leads without piling on personal questions
- SSO options sit alongside the manual fields, so users with corporate Google or Microsoft accounts can skip typing entirely

## Best Practices for High-Converting Lead Generation Forms

Looking through 30 examples is useful, but I wanted to pull out the patterns that show up again and again across the strongest forms.

These are the [form design best practices](https://wpforms.com/form-design-best-practices/) that show up across the strongest examples on this list and that I rely on when I’m helping someone build or fix a lead capture form.

### Match Field Count to the Value of the Offer

The “fewer fields = more leads” rule is mostly true, but it has a ceiling. A newsletter signup with three fields will outperform one with seven, every time.

A demo request with three fields will underperform one with six, because the sales team can’t tell which leads are worth calling.

Pick the field count that matches the offer. Newsletter signups can run on one or two fields. Lead magnets fit comfortably in three to four.

### Use Multi-Step Forms for Anything Longer than Five Fields

Long forms feel long because users see the whole list at once. Break them into pages and the same number of fields suddenly feels manageable.

I’ve seen multi-step forms outperform single-page versions of the same form by 20 to 30 percent on conversion rate, just from the perceived-effort change.

If your form has six or more fields, build it as a [multi-step lead form](https://wpforms.com/how-to-create-a-multi-step-lead-form/) with a progress bar. The progress bar is doing real work here. It tells the user how close they are to done, which keeps them moving.

![Multi part form published](https://wpforms.com/wp-content/uploads/2018/07/multi-part-form-1024x667.png)### Make the CTA Do Real Work

“Submit” is the worst CTA copy in the world because it tells the user nothing about what happens next. Replace every “Submit” button on your forms with action-specific copy:

- “Start My Free Trial”
- “Get the Guide”
- “Talk to an Expert”
- “Send My Question”

The principle behind [submit button best practices](https://wpforms.com/wpforms-submit-button-best-practices/) is to write the CTA as a first-person continuation of what the user just decided. They came to your page to do something, and the button is the verb of that action.

![Submit button text editing](https://wpforms.com/wp-content/uploads/2025/01/submit-button-text-edit-1024x851.png)### Build Trust with Social Proof Above the Form

People convert at higher rates when they see other people already trusting you. The cleanest way to add this is a number, a logo strip, a testimonial, or a star rating placed directly above or beside the form.

Morning Brew’s “Join 4M+ readers” line is a great example. So is Shark’s “1,000+ households trust us” framing. There are [more examples of social proof](https://wpforms.com/brilliant-examples-of-social-proof-for-your-sites-forms/) you can borrow patterns from, but the core idea is to put the proof where the user will see it before they decide whether to type.

### Design the Page Around the Form

The single highest-impact change you can make to a lead form is to put it on a page with no other goals. Strip out the nav menu, footer links, and sidebar so the visitor’s only options are to fill in the form, close the tab, or read the value prop.

This is what most of the examples above have in common. Daily Harvest, Netflix, OkCupid, Peloton, and Figma all use distraction-free landing pages for their highest-intent forms.

If you want to follow the same pattern, our guide on [landing page best practices that convert](https://wpforms.com/best-landing-page-best-practices-that-convert/) walks through the structural choices that matter most.

[![The WPForms homepage](https://wpforms.com/wp-content/uploads/2024/08/WPForms-Website.png)](https://wpforms.com/pricing/)### Build for Mobile First

For a lot of businesses, more than half of all form submissions come from a phone. If your form looks broken or clunky on mobile, you’re losing leads from the first second.

A few practical rules I follow on every form I build:

- **Use responsive forms**: A responsive form adjusts to different screen sizes automatically. Most modern form builders are responsive by default, but it’s still worth testing on a real device. Run your form through a [form testing checklist](https://wpforms.com/form-testing-checklist/) before publishing.
- **Single-column layout**: Multi-column forms tend to add horizontal scroll on narrow screens. Top-to-bottom flow is what mobile users expect.
- **Short, action-oriented CTA copy**: Long button text wraps to multiple lines on mobile. Keep CTAs to two or three words. “Let’s chat” beats “Submit Your Inquiry to Our Team”.

WPForms is mobile-friendly by default, so most of this happens automatically when you build with it.

### Plan the Follow-Up before the Form Goes Live

A lead form starts a conversation. The real conversion happens after the email lands in your list, so what you do with that email matters as much as the form that captured it.

Decide upfront whether you’re using [single or double opt-in](https://wpforms.com/single-optin-vs-double-optin/) for newsletters. Single opt-in grows the list faster but pulls in more low-quality emails.

Double opt-in slows growth but raises engagement and deliverability. There’s no universal right answer, but there is a wrong one, which is to skip the decision and let the default win without thinking about it.

### FAQs on Lead Generation Forms

Lead generation forms are a popular topic among readers, especially when it comes to building high-converting lead capture pages. Here are answers to some of the most common questions about lead gen forms and how to use them.

#### How do you optimize for lead generation?

You’ll see a few themes of effective tactics on this list of lead form examples. Many of these examples use a combination of some of these simple form optimization tricks:

- Use a compelling call to action button
- Offer a discount or lead magnet
- Optimize your pre-existing content as a lead magnet
- Create distraction-free [landing pages](https://wpforms.com/best-landing-page-plugins-for-wordpress/)
- Make multi-step forms if you have a long form
- Try gamified [discount wheel popups](https://wpforms.com/best-discount-wheel-popup-plugins/)
- A/B test one variable at a time, like CTA copy or field count, so you know what actually moved the needle

#### What should be included in a lead generation form?

A good lead generation form includes only the fields you actually need to qualify or follow up with the lead. At minimum, that’s a name and an email address.

For higher-intent offers (demo requests, sales inquiries), you’ll usually add a company name, role, and one qualifying question.

Beyond fields, the form should include a clear value prop near the top so the visitor knows what they’re getting, social proof (a number, a logo, or a testimonial), an action-oriented CTA button, and a brief “what happens next” line under the submit button to set expectations. Adding a privacy or consent checkbox keeps you compliant with GDPR and CCPA.

#### What are the best lead generation tools?

There are a lot of high-quality products out there, but for WordPress users, [WPForms](https://wpforms.com/) is one of the easiest ways to build lead generation forms without writing code.

We’ve also rounded up the [best WordPress lead generation plugins to grow your email list](https://wpforms.com/best-wordpress-lead-generation-plugins-to-grow-your-email-list/) if you want to see how WPForms compares with other options. WPForms includes ready-made form templates to get you started faster.

On top of that, WPForms integrates with all the [best CRM](https://wpforms.com/top-crms-for-your-small-business/) and [email marketing services](https://wpforms.com/how-to-create-an-email-newsletter/), which makes it easy to capture and manage your leads. Plus, WPForms offers a 14-day money-back guarantee.

#### What is an example of a B2B lead generation form?

An example of a B2B lead generation form might be a simple online form on a company’s website. This form typically asks for basic contact information and some relevant business details.

For instance, it might include fields for the person’s name, company name, job title, business email, phone number, and perhaps a section for comments or specific inquiries.

The purpose of this form is to collect information from potential business clients or partners who are interested in the company’s products or services. For instance, you can check out the [lead generation tips](https://wpforms.com/mortgage-lead-generation/) for those in the mortgage industry.

#### How many fields are there in a lead generation form?

The number of fields in a lead generation form can vary, but it’s generally recommended to keep it concise yet informative. Typically, a form might have around 5 to 10 fields. These often include essential contact information like name, email, phone number, company name, and role in the company.

Additional fields might be added depending on the specific needs of the business, such as industry, company size, or specific interests related to the company’s offerings. The key is to balance the need for information with the user’s ease and willingness to provide it.

#### Which are the 4 steps of the lead generation process?

The lead generation process can be broken down into four main steps:

- **Step 1, Attraction.** This involves drawing potential leads to your business. This can be done through various marketing strategies like content marketing, SEO, social media marketing, or paid advertising.
- **Step 2, Conversion.** Once potential leads are attracted to your platform, the next step is to convert them into leads. This is typically done through lead generation forms where interested individuals can leave their contact details.
- **Step 3, Nurturing.** After collecting information, the nurturing process begins. This involves maintaining communication with the leads through emails, newsletters, or personalized follow-ups, providing them with valuable information.
- **Step 4, Closing.** The final step is converting these nurtured leads into customers. This involves making sales pitches, providing tailored solutions, or offering special deals based on the information and relationships developed in the nurturing phase.
